Initial Appointment Guidelines
- Book an appointment
- Complete the patient intake form
- Prepare the following if applicable:
- A valid CDM/EPC letter from your GP
- Valid Medicare card
- Current copy of NDIS plan and Plan Manager details
- Previous relevant medical reports completed by health professionals or school
- Arrive 10 minutes prior to your appointment
- Ensure patient intake form is completed
- Please understand that delays may happen
- Initial appointments normally take one hour. Clients can take a break as necessary.
- We need the following information from you so we can best support your needs:
- NDIS goals
- Any relevant medical reports
- List of current medications
